Ongoing Quality Assurance Reviews

Our internal Quality Management Committee conducts quarterly reviews of clinical charts, staff & billing documentation, and service outcomes. These reviews help us identify strengths, address gaps, and implement agency-wide improvements. A sample of client records from each program is reviewed monthly, with documented action steps taken within 10 business days when deficiencies are found. This allows us to maintain high performance across all areas of care and administration.

Incident Reporting & Risk Mitigation

We maintain a strict protocol for reporting and investigating client-related incidents, including medical emergencies, behavioral crises, medication reactions, and safety concerns. All incidents are reviewed by leadership, reported to AHCCCS within regulatory timelines, and logged in a secure incident reporting system. We use incident data to proactively mitigate future risks and refine care practices in real time.

Compliance with ADHS & Payer Requirements

Our clinical and administrative practices align with Title 9, Chapter 10 regulations from ADHS, as well as payer-specific guidelines from Medicaid, AHCCCS, and private insurers. From billing documentation to treatment plan audits and staffing credentials, every aspect of our agency is maintained for audit-readiness and policy alignment. We conduct regular staff training on HIPAA, documentation integrity, and licensure requirements.

Client Feedback & Satisfaction Monitoring

We value the voice of clients. Every quarter, we distribute a formal satisfaction survey to assess progress, identify service gaps, and measure overall experience. Results are compiled, reviewed by executive leadership, and used to inform improvements in programming, communication, and care delivery. If concerns are raised, our Administrator meets with involved staff and clients to implement corrective actions.
